UT Math REU Application Form

According to NSF rules, participants must be US Citizens or Permanent Residents. Please do not inquire about our program if you are not a US Citizen or Permanent Resident; although some other programs may have options for you, our program does not.
Please read this information carefully before clicking "Apply Now" below. The application form is a PDF file, which can be read and filled out using Adobe Acrobat Reader. Most computers already have this software installed and will correctly display the form. If you do not have Adobe Acrobat reader, or have an old version, it may be downloaded for free at http://get.adobe.com/reader/
Unfortunately, Acrobat Reader will not allow you to save the data in the form. To save you need Acrobat, which is definitely not free, or some other software that will allow you to produce a PDF. Many campuses have site licenses for Acrobat which you may be able to use. Therefore I recommend that you view the form, and perhaps print a blank copy, gather the information that you will need, then fill out the form and print it immediately. You will be required to enter a short statement of interest; I recommend that you write this in a separate word processor and paste it into the form, to avoid losing what you have written if something goes wrong with the form.
Except where noted, all information on the form is required. This information will only be made available to faculty and staff in the math department who are directly involved in the program. The names and colleges of participants will be publicly available, as will some materials involving student projects. By submitting the application form you confirm that the information is correct and that the information you have submitted may be used as described here.
You should ask two of your math professors to write letters of reference for you. You will list them and their e-mail addresses on the form, but you must also ask them to write the letters! You should refer them to the letters web page (also the link "Letters of Reference" to the left) for more information about submitting the letters.
You must submit the completed form and a transcript (unofficial is OK). There are two ways to do this:
(1) If you have Adobe Acrobat or other software that enables you to print the form to a PDF (check to see if "print to PDF" or "Adobe PDF" is an option when you select print), then you may e-mail the PDF and electronic version of your transcript to cplaut@math.utk.edu. The subject should be "REU Application" followed by your last name. Before sending the form, open it separately with Acrobat Reader to check that it actually has your data in it.
(2) If you are unable to produce a PDF file containing the completed application then print a paper copy and mail or fax it and the transcript to the address or fax number on the application form. Yet another option is to scan the paper copy and e-mail it as described in part (1). If you fax the transcript separately, include a cover letter that mentions the REU program, so the staff knows to give it to me.
Unfortunately we do not have the resources to acknowledge receipt of applications. However, there is a built-in system of checks: the only way we will be unaware of your existence is if your application and both of your professor's letters are somehow not received. In case of partial application we will review what we have and make a decision whether to seek the missing parts.
