Doctoral Forms Packet
Doctoral students are required to submit additional forms with the dissertation. This packet of forms include the Survey of Earned Doctorates, The Doctoral Dissertation Agreement Form (with a copy of the title page attached), and the Abstract form (with a copy of the abstract attached). Please note that the second half of the last page of the Doctoral Dissertation Agreement Form, under the heading "Authorization to apply for registration of copyright," is NOT to be filled out; it is superseded by the separate Copyright Registration Form. Doctoral students wishing to have Proquest register the dissertation with the U.S. Copyright Office on their behalf may submit this Copyright Registration Form, including a $65 certified check or money order made out to PQIL. This is the only form in the packet that is optional; the others are mandatory.
Copyright
Under U.S. law, a document is protected by copyright upon its creation. The author(s) do not need to take any additional steps or pay any fees to have copyright protection; the document (images, ec.) is considered the intellectual property of the author(s). Doctoral students are entitled to include a copyright page (following the title page), without paying additional fees. (See the Guide to the Preparation of Theses and Dissertations for details.) Registering your copyright with the U.S. Copyright Office (per the form, below) does not change a document's copyright status, but it may provide additional recourse and access.
