Test Agreement

In order to utilize the exam proctoring assistance from the Office of Disability Services, I agree to do the following:

  1. Complete an ODS Letter to Professor Request form each semester, in order to receive accommodations.
  2. Deliver the official Faculty Accommodation Letter to professors and discuss accommodations that are being requested.
  3. Complete a blue Test Request Form, which is located in the Office of Disability Services Testing Center.
    • Complete the student section and then have my professor/instructor complete the exam schedule and information for the entire semester.
    • Return the completed form to ODS, no later than 5 business days before the first exam date.
    • I understand that ODS is not able to reserve a time nor space for me without completing the proper paperwork in a timely manner.
  4. Take the exam at the scheduled time. If I am unable to take the exam at the scheduled time, I will notify ODS and the professor. The professor will have to contact ODS to approve the change and to provide an alternative date and/or time to complete the exam.
  5. Follow up with my ODS Coordinator to discuss any difficulties I am having with implementing my testing accommodations.

Cheating will not be tolerated! If I am caught cheating, my exam and unapproved exam materials will be confiscated. The matter will be turned over to my professor and Student Judicial Affairs.
I understand that failure to comply with the rules and regulations of the Office of Disability Services may result in a forfeiture of services for an academic year.
Any and all exceptions made in the Testing Center must have prior approval from the Student Support Services Coordinator and/or Director of Disability Services.

You must accept this agreement before you may move on.
