J-1 Forms
- J-1 Request
When a UT department has invited an international scholar for a visit, the J-1 visa is often the most appropriate visa for the visitor. This form should be completed by the host department. This request form should not be used for students. The J-1 visa is also not appropriate for tenure-track or other long-term employment (longer than 5 years total). - Request for J-1 Extension (Department Form A)
A host department should fill out this form to request an extension of J-1 program for an international scholar or visitor. Note that the maximum length of time permitted for a J-1 scholar is normally three years from the date of entry to the U.S. - Request for J-1 Extension (Scholar Form B)
An international scholar requesting an extension of J-1 status should complete this form and submit to host the department. The department then completes Form A above and submits both applications together. - Short Term Activities Payment
Normally, a visiting professor, researcher, or short-term scholar with J-1 status may be paid only by the sponsoring institution. Thus, in most cases, J-1 scholars sponsored by The University of Tennessee, Knoxville may not receive payments from other U.S. institutions, unless arrangements have been made for this before the scholar arrives. However, J-1 scholars may apply to their sponsor for permission to receive some payment from other institutions for short-term activities classified as “occasional lectures or consultations”. This information explains what types of payments can be authorized and how to apply for the authorization. - Address Change Notification
U.S. law requires all J-1 Exchange Visitors to inform their program sponsors of their U.S. address within 30 days after arriving in the U.S. and within 10 days after any move to a different U.S. address. This address change must then be entered by your Program Sponsor into the SEVIS database. (Student and Exchange Visitor Information System). Failure to notify the Program Sponsor of a change in address will result in the termination of the Exchange Visitor’s program. - J-1 Scholar Request for DS-2019 for Dependents
Spouses and/or children under the age of 21 who wish to accompany or join the principal exchange visitor (J) visa holder in the United States for the duration of his/her stay require J-2 visas. The visa application procedure is the same as that for a primary visa applicant. If your dependents wish to apply for J-2 visas, they will need separate DS-2019 forms issued by the UT Center for International Education. To request forms for your dependents, complete this Request Form and then make an appointment with an advisor in the CIE International Scholar Services office to discuss plans for their arrival. (Note: If your DS-2019 was not issued by The University of Tennessee, then you will need to contact your sponsor to inquire about dependent visas.) - Student Request Form (UT STUDENTS ONLY)
This form is used by a J-1 student to request a DS-2019, a travel signature, or an extension of the student's J-1 program. - Academic Advisor J-1 Extension Request Form
This form is used by a student's academic advisor to provide information in support of the student's request for a J-1 program extension. - Certification of Academic Status
This form should be completed and signed by the intern’s Academic Advisor or other representative at his/her home university. - DS-7002: Training / Internship Placement Plan
- English Proficiency Form for Prospective J-1 Intern
If the required interview was conducted in English by the UTK host professor, this form can be filled out by the same person (Section 1). Otherwise, the English Proficiency Form may be completed by an English Instructor at an academic institution or an English Language School (Section 2). A third option is to attach a copy of a TOEFL score which would satisfy UTK undergraduate admission requirements. - Interview Report Form for Prospective J-1 Intern
This form must normally be completed the intern’s host professor at The University of Tennessee, Knoxville, before the DS-7002 and the J-1 Request Form are completed. In some cases, the interview can be conducted by a representative of the student/intern’s home university if there is a written agreement with UTK. - J-1 Request Form (Intern)
This form is used to initiate paperwork for a J-1 Visitor coming in the Intern category.

This form should be completed by the UT department or organizational unit wishing to employ an H-1B employee. - H-1B Applicant Information
This form is used to provide information to the Center for International Education about a UT employee (current or prospective) who will be the beneficiary of an H-1B petition. - H-1B Actual Wage Calculation Form
This form is used to comply with the U.S. law requiring employers to document that an H-1B employee will be paid the "actual wage" of his/her position. - H-1B Request Amendment
This form should be used when a hiring department wishes to change information provided on a recent "Request for H-1B Petition."
