Address Change Notification for J-1 Students

U.S. law requires all J-1 Exchange Visitors to inform their program sponsors of their U.S. address within 30 days after arriving in the U.S. and within 10 days after any move to a different U.S. address. This address change must then be entered by your Program Sponsor into the SEVIS database. (Student and Exchange Visitor Information System). Failure to notify the Program Sponsor of a change in address will result in the termination of the Exchange Visitor’s program.

If you are sponsored by UT, please use the form below to inform the Center for International Education of any change in address. The form can be mailed or hand-delivered to the Center for International Education, 1620 Melrose Avenue, The University of Tennessee, Knoxville, TN 37996-3531.

The form can also be faxed to 865-974-2985 ONLY if it is not possible to deliver the form by mail or in person in time.

If you have a different program sponsor (such as ISEP), please contact your sposonr directly to inform them of any address change.
