This form was developed by the AAUP to provide the institution with information about 
the performance of administrators. Your response will be kept strictly anonymous. You 
may use this form to evaluate up to seven people; photocopies will not be accepted. If 
you do not feel you have adequate information on which to answer a question, please 
write "NA" on the appropriate line(s). DO NOT EVALUATE YOURSELF.

Answer the first six questions (A-F) using the following scale:
1=strongly disagree; 2-mildly disagree; 3=mildly agree; 4=strongly agree. Answer
