1. Hospital

This is the most probable location for these decisions - for several reasons.

• The majority of Americans die in a hospital - even those involved in auto accidents or other forms of trauma are generally taken to the hospital where death is declared.
• Hospitals are where people are more likely to be receiving the kinds of treatments that decisions might be made to withhold or withdraw
• Hospitals are where there are the kinds of professionals who are more likely to agree to and/or make such decisions.

Whether most people would prefer to be in some other setting at the end of their life is another question entirely.
