Undergraduates
The following forms are available for undergraduate English majors:

Graduate Students
The following forms are available for individuals applying to the English Department graduate program. You must submit all three forms. See the Graduate Studies page "How to Apply" for a complete description of the application process and information about where to send each form.
- Application to UT Graduate School (this form must be sent to the UT Office of Graduate Admissions and Records)
- Application for Graduate Appointment (this form must be sent to the English Department)
- English Department Recommendation Form (this form must be sent to the English Department)
