You can send an e-mail request for an application packet by e-mailing us at hinkle@utk.edu. (Please be sure to include your mailing address.)
You can also complete your application using the materials available on our website. Print out copies of the application forms by clicking on the following links. You need all three forms to apply. You will also need to complete the online application to the Graduate School. See "How to Apply" for full instructions on the application process.
