Registration Form

Registration Fee:
Registration Fee (forms received by July 9): $ 40
Late Registration Fee (beginning July 10): $ 50

Payment:
All payments by mail should be either a check or money order in U.S. dollars made payable to WSO and must accompany the registration form. Cash will be accepted at the workshop. No credit cards please. Christine Reel, Treasurer for Wisconsin Society for Ornithology (WSO), is managing the registration for the workshop.

Refunds: No refunds will be accepted after July 9.

Important Deadlines:
cJuly 9-Last day to register for the $40 registration fee.
gggggggg-Last day for refund of your workshop payment.
gggggggg-Last day that rooms at the Lodge at Crooked Lake are being held for the workshop. See "Accommodations" bbbbbbbbfor more information.
vJuly 10-Registration forms received on this date or later will require a $50 registration fee.

Registration after July 25:
Availability of workshop meal tickets, a seat on the bus for Thursday's field tour, and certain registration materials (abstracts, field tour guide, etc.) will be available for a limited number of attendees who register after July 25 and will be given out on a first-come first-serve basis. All other late registrants will not be able to participate in workshop meals, ride a coach for the field tour, and will not be guaranteed a complete set of registration materials.

Meals:
Your registration includes the Wednesday BBQ lunch, Wed. Evening Fish Boil & Social, and Thursday Bus Tour lunch. A continental breakfast is provided for folks lodging at the Lodge at Crooked Lake. No meal or activities are scheduled for Thursday evening. Attendees may wish to have dinner at one of the area's fine eating establishments. Refreshments will be available at the breaks.

Extra Meal Tickets:
Extra meal tickets are available for guests and family members to attend the Wed. BBQ lunch and the Wed. Evening Fish Boil & Social. You must request extra meal tickets by July 25.

Special Food Needs:
Please explain your special food needs on the registration form. Vegetarian options are available for all workshop affiliated meals. If you have multiple person or complex special food needs, please contact Christine Reel directly so that we can better accommodate you.

Post-Workshop Field Trips on Saturday, August 13:
Please refer to the home page for field trip decriptions. For the St. Croix River Canoe Trip, please indicate the number of canoes you intend to rent for $27.00 each. Keep in mind that each canoe seats 3 people but the $27.00 fee only covers 2 people; add $6.00 to the rental fee per canoe if the 3rd person is 12-years-old or older. Children under 11-years-old are free. You may bring your own canoe or kayak and equipment but you must arrange your own shuttling. For the Crex Meadows Bird Hike, there is no fee. Please indicate the number of people who plan to attend the bird hike. A box lunch is available on the registration form for $7.50 per person and must be ordered by July 25.

T-shirts:
Workshop T-shirts can be ordered on the registration form in either women's or men's style for $15.00 (includes tax) each. The shirts will be forest green, 100% cotton with the workshop logo embroidered toward the upper left shoulder. T-shirt orders must be received by July 25 and will not be sold at the workshop.
