The capstone ACCLAIM mathematics course to be taught by Michael Mays will be listed in the WVU Summer II schedule of courses as the WVU
course Math 534. ACCLAIM students should apply for admission to WVU by completing the WVU ACCLAIM Graduate Application form, available online at:

http://www.arc.wvu.edu/admissions/pdf_forms.html

The form should be printed off, completed, and mailed back to:

Once Sandy has all the applications back she will arrange for them to be processed at WVU and for the students to be registered for Math 534.

*It is very important for all of the applications to be returned as soon as possible.*

The application fee will be waived. Tuition cost is the in-state fee for WVU courses, $220 per credit hour, of which ACCLAIM pays half.
